CAMP REGISTRATION

My child has my permission to attend a Texas QB event during 2023. I certify that I am the parent or legal guardian of the player registering for the Texas QB event. I certify that my child has been examined by a licensed physician within the last 12 months, and is able to participate in all football program-related physical activities. I understand that my child may come in contact with other people & objects, and I hereby waive all rights to future claims related to such contact or injuries sustained as a result of such participation or contact. I agree to assume any and all risks associated with my child's participation in a Texas QB event and release the host site, any and all representatives or members of host site, Texas QB, Jason Lovvorn, David Robinson, and all assistants of Jason Lovvorn from any and all liability related to injury or illness my child may sustain at a Texas QB event.
